Yorkshire Ambulance Service Admits Breach Over 111 Triage in Death of Natasha Hewitt

The trust’s concession that earlier care would probably have saved her underpins a negligence settlement process.

  • The service accepted a breach of duty and acknowledged that, on the balance of probabilities, timely diagnosis and treatment would have saved the 35-year-old mother.
  • During an NHS 111 call she reported a sudden severe headache, dizziness and vomiting but was advised to contact her GP instead of being sent for urgent hospital assessment.
  • Roughly a day later she was taken to hospital, diagnosed with cerebral venous sinus thrombosis and died two days after neurosurgery on 18 December 2022; she had a history of placental and lung clots.
  • An earlier inquest found a missed opportunity for earlier referral and concluded that neglect contributed to her death.
  • Irwin Mitchell, representing her husband, says talks are progressing toward a settlement as the trust issues an apology and points to completed investigations and service improvements.
