Walmart Adds Same-Day Delivery of Refrigerated Prescriptions With Grocery Orders

The expansion targets soaring specialty drug demand that is reshaping pharmacy costs.

Overview

  • Walmart now includes refrigerated and reconstituted medicines such as insulin, GLP-1 therapies, and pediatric amoxicillin in same-day pharmacy delivery bundled with groceries in a single online order.
  • The company says the added cold-chain capability lets it deliver about 90% of prescriptions, and that refrigerated or reconstituted items account for roughly 30% of its pharmacy sales.
  • Since launching same-day pharmacy delivery last October, Walmart reports fulfilling more than 4 million orders, with some delivered in as little as nine minutes.
  • Walmart claims it is the first retailer to deliver refrigerated and reconstituted prescriptions together with groceries, while rivals like CVS and Walgreens already deliver specialty drugs and Amazon offers same-day delivery with separate packaging for cold items.
  • Benefits consultant Aon identified GLP-1 prescriptions as the single biggest driver of employer health costs for 2025, and Walmart previously cited GLP-1s as a key contributor to recent pharmacy growth.
