UK Screening Data Support Lung Cancer Checks Up to Age 80 for Surgically Fit Patients

New outcomes data from two UK programs could guide decisions on upper age limits for screening.

Overview

  • At IASLC WCLC 2025, researchers reported outcomes from 574 screen-detected lung cancer cases in the Yorkshire Lung Screening Trial and the North & East Manchester Lung Health Check.
  • Of these cases, 190 involved patients aged 75–80, and their stage distribution matched that of patients aged 55–74.
  • Curative-intent treatment was delivered in 87% of cases overall, with surgical resection less common in the older group (42% vs 58%, p<0.001).
  • All-cause mortality was higher among patients aged 75–80 (hazard ratio 1.54), with four-year mortality of 44% compared with 34% in those aged 55–74.
  • Among patients who underwent surgery, four-year mortality was similar between age groups (16% in 75–80 vs 18% in 55–74; HR 1.00), supporting selection based on surgical fitness rather than age alone.
