UK Launches Investigation into Systemic NHS Maternity Failures

  • The two-part review will first examine up to ten of the worst-performing maternity and neonatal units before conducting a system-wide assessment.
  • A National Maternity and Neonatal Taskforce chaired by the Health Secretary will bring together experts and affected families to guide reforms.
  • A new digital platform will flag safety concerns by November and an anti-discrimination programme will target ethnic disparities in care.
  • The inquiry is due to report to ministers by December 2025 with recommendations to improve staffing, facilities and patient safety.
