- The UL Research Institutes/Fire Safety Research Institute released a 723–724 page state-commissioned report on Oct. 2, 2026 that concluded the January 2025 Palisades and Eaton fires were caused by multiple, interlocking system failures rather than a single mistake.
- Investigators documented major emergency communications and incident-command breakdowns, including 911 calls temporarily routed through a captain’s personal cellphone and firefighter units going “dark,” which reduced situational awareness and hampered evacuations.
- The report found the built environment and neighborhood conditions played a substantial role in fire spread, saying dense, older structures and structure-to-structure pathways turned homes into fuel and made individual home hardening insufficient on its own.
- The Los Angeles Fire Department acknowledged missteps tied to the Lachman burn and early Palisades response, said it accepts the findings, and reported implementing many internal changes while litigation and an arson retrial continue.
- Authors issued 266 specific recommendations covering governance, evacuation planning, communications equipment and post-containment checks but warned that missing agency records left assessment gaps and that a roughly $16 billion shortfall in recovery funding and coordination will obstruct meaningful implementation.
