Spain Orders Objector Registries as Report Exposes Wide Gaps in Public Abortion Care

The government set a three-month deadline for four regions to create conscientious-objector registries, signaling legal enforcement following Madrid’s hesitation.

Overview

  • The Health Ministry reported that 21.25% of abortions were provided in the public system in 2024, up from 14.3% in 2019 after the 2023 reform.
  • Provision remains highly uneven, with no public procedures in Extremadura, Ceuta and Melilla and only about 0.5% in Madrid and 0.2% in Andalucía, while Cantabria, Galicia, La Rioja and Navarra exceed 60% public provision.
  • Andalucía and Madrid account for nearly half of all abortions nationwide, intensifying the impact of their low public provision on the national share.
  • The government issued formal orders to Aragón, Asturias, the Balearic Islands and Madrid to establish conscientious-objector registries within three months to reduce referrals to private clinics.
  • Madrid regional president Isabel Díaz Ayuso declined an immediate response to the order, and Health Minister Mónica García warned the state will use all available legal and administrative tools to ensure compliance.
