- Security video released this week shows the operator appearing to nod off moments before passengers were jolted as the train exited the Sunset Tunnel at about 50 mph.
- The Sept. 24 inbound train passed the Duboce Avenue/Noe Street stop and halted on Duboce near Walter Street, with riders thrown and at least one concussion reported, and no derailment.
