SFMTA Blames Operator Fatigue After Video Shows N‑Judah Racing at 50 MPH

The operator is off driving duty pending internal protocols, with location‑based speed controls in development.

Overview

  • Security video released this week shows the operator appearing to nod off moments before passengers were jolted as the train exited the Sunset Tunnel at about 50 mph.
  • The Sept. 24 inbound train passed the Duboce Avenue/Noe Street stop and halted on Duboce near Walter Street, with riders thrown and at least one concussion reported, and no derailment.
  • SFMTA’s investigation determined the cause was operator fatigue after inspections confirmed the braking system, track and related infrastructure performed as designed.
  • The operator has been placed on nondriving status in line with agency protocols and the labor contract, and the operator’s identity has not been released.
  • The agency is reinforcing fatigue‑awareness training and working with Siemens on software to limit speeds at specific locations, and the train was in manual mode during the incident.
