Recall Issued for Forward Farms Ground Beef in Six States Over E. Coli O26

Routine USDA testing detected E. coli O26 in a Dec. 16 batch, triggering an active recall.

Overview

  • Mountain West Food Group is recalling about 2,855 pounds of raw ground beef following FSIS confirmation of contamination.
  • The affected item is a 16-ounce vacuum-sealed package labeled "Forward Farms Grass-Fed Ground Beef" with EST. 2083 and a use-or-freeze-by date of 01/13/26.
  • The product was produced on Dec. 16, 2025, and distributed for retail sale in California, Colorado, Idaho, Montana, Pennsylvania and Washington.
  • No illnesses have been reported to date, and consumers are advised to discard the product or return it and to cook ground beef to an internal temperature of 160°F.
  • FSIS is conducting recall effectiveness checks and plans to post distribution updates to help ensure removal from stores and home freezers.
