- Mountain West Food Group recalled about 2,855 pounds of raw ground beef sold under the Forward Farms brand following FSIS sampling results.
- The affected product is a 16-ounce vacuum-sealed package labeled "Forward Farms Grass-Fed Ground Beef" with "USE OR FREEZE BY 01/13/26" and establishment number "EST 2083."
- Production occurred on Dec. 16, 2025, with distribution to California, Colorado, Idaho, Montana, Pennsylvania and Washington for retail sale.
- FSIS advises consumers to discard the product or return it to the place of purchase and to cook all ground beef to an internal temperature of 160°F.
- E. coli O26 is a Shiga toxin–producing strain that can cause diarrhea and vomiting, with symptoms sometimes appearing 28 to 34 days after exposure, and higher risk for young children, older adults and immunocompromised people.
