Phillip Schofield, former anchor of ITV's 'This Morning', refused to participate in an external review into his relationship with a young colleague, citing a 'risk to his health'.
The young colleague, who was more than 30 years Schofield's junior, also declined to participate in the review.
Of the 48 people interviewed during the review, only one junior employee had knowledge of the affair, but did not report it at the time.
ITV was unable to uncover evidence of the relationship until Schofield's admission in May 2023, despite considerable efforts.
The review recommends that ITV set out clear guidelines for its talent to ensure good behavior, even by household names.
