One Worker Killed in Yarmouth Trench Collapse as Two Are Injured in Hourslong Rescue

An on-site OSHA investigation now scrutinizes Revoli Construction’s safety record.

Overview

  • The collapse occurred around 8:50 a.m. Tuesday at 152 S. Shore Drive in South Yarmouth near the Skipper Chowder House.
  • Specialized teams shored the trench with hydraulic jacks and boards and used pulleys, ladders, ropes and a drone during a prolonged operation.
  • One worker was recovered deceased at about 2:45 p.m., another was flown to a Rhode Island trauma center, and a third self-extricated and was taken by ambulance.
  • The site is part of a multi-year sewer project for which Revoli Construction holds an approximately $18 million town contract.
  • OSHA is investigating on site, and records show Revoli faced fines in 2023 and 2024 for safety hazards, including failures to protect against cave-ins, as police asked the public to avoid the area.
