Overview
- The collision over Cabazon occurred on Aug. 6, 2023, killing Cal Fire Assistant Chief Josh Bischof, Capt. Tim Rodriguez, and contract pilot Tony Sousa aboard a Bell 407.
- The Skycrane entered the fire traffic area at about 4,000 feet despite a 2,500-foot clearance, then descended aggressively into the Bell, which was level at its assigned altitude.
- Investigators reported the Skycrane failed to establish proper supervisor clearance before entering restricted airspace, with mountainous terrain contributing to radio reception problems.
- Surveillance footage showed the Skycrane’s right wheel and tire assembly struck the Bell 407, triggering an explosion and the separation of major components of the smaller helicopter.
- The report notes cetirizine use by the Bell pilot did not contribute to the crash, Cal Fire has implemented enhanced training and communication protocols with strengthened after-action reviews, and wrongful-death lawsuits for the victims are proceeding using the findings.
