Overview

  • Nottinghamshire Police opened the probe on June 2 to determine whether organisational failings at Queen’s Medical Centre and Nottingham City Hospital breached the corporate manslaughter law
  • The inquiry forms part of Operation Perth, the largest independent maternity review in NHS history examining around 2,500 neonatal deaths, stillbirths and injuries
  • Donna Ockenden’s review team has already submitted 200 case files to detectives, with further referrals expected as investigations continue
  • Affected families have welcomed the criminal investigation as a step toward justice but criticised the lack of individual accountability under corporate manslaughter charges
  • NUH leaders have pledged full cooperation with police and committed to boosting midwife numbers, strengthening training and improving maternity services
