Norfolk Call Handler Receives Warning for Mishandling Fatal 999 Call

The warning follows an IOPC finding that the handler failed to log a risk assessment after a caller warned he had a knife.

Overview

  • A June 26 misconduct hearing issued the call handler a final written warning valid for 12 months.
  • IOPC investigators found the handler did not record a risk assessment and attempted to divert the January 19, 2024 call under the Right Care, Right Person scheme.
  • During the call Bartlomiej Kuczynski warned he had a knife and said he had ‘lost the plot’ shortly before killing his two daughters and their aunt.
  • Police did not arrive at the Allan Bedford Crescent property until an hour after the 999 call, responding only after a bystander contacted officers.
  • A full inquest into the Costessey deaths is scheduled for 2026 pending a Domestic Homicide Review, with an NHS serious incident review also under way.
