NICE Proposes Screening and Annual NHS Reviews for PMOS

Published Wednesday, the draft guidance aims to prompt earlier diagnosis by recommending tests for people with irregular or absent periods who show signs of raised androgens.

Overview

  • The National Institute for Health and Care Excellence published draft guidance on Wednesday recommending investigation for polyendocrine metabolic ovarian syndrome when people have irregular or absent menstrual cycles plus signs of raised male hormones.
  • The draft asks that anyone diagnosed with PMOS be offered a yearly health review to monitor symptoms, assess treatments and check for long-term risks such as type 2 diabetes and cardiovascular disease.
  • NICE estimates PMOS affects three to four million people in the UK and says the condition is often underdiagnosed and inconsistently managed, which the guidance seeks to address through clearer diagnostic criteria.
  • The document includes advice on fertility care, mental-health support, lifestyle measures and medical options for symptoms but rules out NHS-funded laser or light therapies for excess hair growth because of limited evidence and possible costs up to about £100 million a year in England.
  • Clinicians and charities have welcomed the draft but warn that NHS capacity, funding and persistent barriers for Black, Asian and other minoritised groups will determine whether the guidance narrows current gaps in care as it moves through consultation.
