NICE Drafts Plan to Add Quick Spine Checks to Routine DXA Scans

The draft guidance promotes a minutes-long vertebral check during DXA appointments to catch silent spinal fractures earlier.

Overview

  • NICE proposes adding vertebral fracture assessment to DXA bone density scans for adults aged 50 and over, using the same machine in the same visit.
  • People with a previous hip or spine fragility fracture, or with two or more fragility fractures from minor falls, would be offered DXA scans automatically.
  • The Royal Osteoporosis Society estimates about 2.2 million people in the UK have undiagnosed spinal fractures, and nearly three million people in England live with osteoporosis.
  • The UK Government has committed 13 additional NHS DXA scanners expected to enable about 29,000 more scans per year.
  • The draft is open for consultation until 23 February 2026, as patient advocates press for treatment pathways and nationwide fracture liaison services to ensure follow-up care.
