New West Health–Gallup Survey Finds Record Anxiety Over Health Costs as 1 in 5 Can’t Afford Prescriptions

The timing coincides with the scheduled Dec. 31 expiration of enhanced ACA subsidies, heightening concern over out-of-pocket costs.

Overview

  • Nearly half of U.S. adults, 47%, say they are worried they won’t be able to afford needed care next year, the highest level since tracking began in 2021.
  • One in five Americans reported being unable to pay for prescriptions in the past three months, and about one-third delayed or skipped care due to cost.
  • State experiences diverge sharply, with Iowa, Massachusetts, Washington, D.C., and Rhode Island ranking highest while Texas, New Mexico, Nevada, and Alaska rank near the bottom.
  • Access and quality vary widely, with 66% in Nebraska saying it’s easy to get care versus 30% in New Mexico and 31% in Nevada, and perceived high-quality care ranging from 79% in Massachusetts to 56% in Texas.
  • The study surveyed 19,535 adults nationwide in June–July 2025, and experts say the findings could add pressure on Congress to act on ACA premium subsidies set to lapse at year’s end.
