Monash IVF confirmed that a woman at its Brisbane clinic gave birth to a baby following the erroneous transfer of another patient's embryo.
The error, attributed to human mistakes, was detected in February when the birth parents sought to transfer their remaining embryos.
Monash IVF has apologized to the families involved and stated it is providing ongoing support during this distressing time.
An independent investigation, led by Fiona McLeod AO SC, has been commissioned to identify additional safety measures and prevent future errors.
The incident has been reported to relevant regulatory bodies, including the Reproductive Technology Accreditation Committee and Queensland ART regulator.
