Mizoram Tops India in HIV Prevalence Even as Prevention Ranking Rises
Officials report stronger prevention performance with new infections falling since 2018.

- Mizoram’s adult HIV prevalence stands at 2.75%, the highest in India against a national average of 0.20%, according to state officials.
- The state improved on NACO/MSACS prevention scorecards, moving from fifth in 2024–25 to third in 2025–26.
- Between April 2024 and November 2025, 3,257 people tested positive from more than 140,000 samples, including 953 women and 179 pregnant women.
- Of the recent cases, 70.4% were sexually transmitted, 27.3% were linked to needle sharing among people who inject drugs, 1.8% were parent-to-child, and 0.8% were from unknown sources.
