Miguel Reis Identified in Yarmouth Trench Collapse, OSHA and DA Investigate

Scrutiny has turned to contractor Revoli Construction, previously cited by OSHA on this sewer project.

Overview

  • Town officials named the worker who died as Miguel Reis, 61, of Fall River, after a trench collapsed during sewer work on South Shore Drive on Nov. 18.
  • Rescuers recovered Reis’s body at about 2:45 p.m. after hours of efforts while the trench continued to cave in during operations.
  • Two other workers were injured: one self-extricated and was taken to Cape Cod Hospital, and another buried to his waist was freed after several hours and flown to Rhode Island Hospital.
  • The collapse occurred around 8:50 a.m. near the Skipper Chowder House, prompting a large multi-agency response that used shoring, pulleys, ladders, ropes, and a drone.
  • OSHA and the Cape & Islands District Attorney’s Office, with Massachusetts State Police detectives, have opened investigations as reports note Revoli’s prior OSHA violations and a $6,950 fine tied to the same project.
