Maintenance Failures Led to Drowning of Two Wheelchair Users in Devon Lake

The incident occurred during a day trip from Burdon Grange care home, with six people thrown into the water.
The South West Lakes Trust, managing the activity center, halted Wheelyboat use and plans to review safety recommendations.
Chief Inspector Andrew Moll highlighted a lack of risk assessment and oversight in operating the boat for vulnerable passengers.
