Lilly Taps Walmart for First Retail Pickup of Discounted Zepbound Vials Through LillyDirect

The cash-pay option targets patients facing coverage limits, sharpening competition with rival GLP-1 retail rollouts.

Overview

  • Starting in mid-November, patients with prescriptions can get single-dose Zepbound vials at Walmart pharmacies nationwide or via home delivery, with Walmart serving as LillyDirect's first in-store pickup partner.
  • Prices are $349 per month for the 2.5 mg starter dose and $499 for all higher doses, the same whether picked up at Walmart or delivered.
  • Under the cash-pay program Lilly provides vials rather than auto-injector pens, and access to higher doses requires enrollment in the Zepbound Self Pay Journey Program.
  • Lilly reports that direct-to-consumer channels accounted for about 35% of new Zepbound prescriptions in Q2 2025 as more patients seek options outside insurance.
  • The Walmart deal expands LillyDirect’s reach through a large national pharmacy network and comes as Novo Nordisk pursues similar retail access for Wegovy with Costco and with CVS/Caremark.
