Leapfrog’s Fall 2025 Hospital Safety Grades Tie Top Marks to Health System Affiliation

The results spotlight Leapfrog’s finding that affiliation with multi-hospital systems is strongly associated with top marks for preventing harm.

  • Leapfrog released its biannual report card for nearly 3,000 general hospitals, using up to 22 patient-safety measures drawn from federal data and its voluntary survey.
  • Ninety-four percent of A-rated hospitals are system affiliated, and 95% of the 358 Straight A hospitals are in health systems, with 90% of all graded hospitals belonging to systems.
  • Utah again ranks first for the share of A hospitals, followed by Virginia, New Jersey, Connecticut and North Carolina, while Iowa, North Dakota, Vermont and Wyoming have none.
  • Health systems with the most A hospitals include HCA Healthcare (51), CommonSpirit Health (49), AdventHealth (29) and Kaiser Permanente (27).
  • Twenty-three hospitals received an F grade, a group that includes facilities in Florida, Illinois, Iowa, Michigan, New York and Texas, as legal challenges to Leapfrog’s methodology continue and the group defends its approach.
