Leapfrog Releases Fall 2025 Hospital Safety Grades, Highlighting Health System Advantage

Leapfrog stresses the link to higher grades is correlational rather than proof that consolidation improves safety.

Overview

  • Nearly 3,000 general hospitals were graded using 22 safety measures drawn from CMS data, adverse event reporting and Leapfrog’s voluntary hospital survey.
  • About 90% of graded hospitals belong to health systems, and 94% of A-rated facilities are system-affiliated; 95% of the 358 straight‑A hospitals are in systems.
  • Utah led state performance for a fifth consecutive cycle, followed by Virginia, New Jersey, Connecticut and North Carolina, while Iowa, North Dakota, Vermont and Wyoming had no A hospitals.
  • Large systems accounted for many top performers, including HCA Healthcare (51 A hospitals), CommonSpirit (49), AdventHealth (29) and Kaiser Permanente (27).
  • Twenty‑three hospitals received an F grade, with ongoing pushback including a Florida federal lawsuit that Leapfrog denies and that is slated for trial in January, as CMS star ratings largely align with the straight‑A and F lists.
