Leapfrog Fall 2025 Safety Grades Show A Hospitals Concentrated in Large Health Systems

The report highlights a correlation with system-level resources, not proof of causation.

Overview

  • Leapfrog released its fall 2025 Hospital Safety Grades for nearly 3,000 U.S. hospitals using 22 measures developed with Johns Hopkins experts and federal data.
  • About 94% to 95% of A-rated hospitals are affiliated with multi-hospital systems, and hospital chains accounted for 90% of hospitals graded and 94% of A grades.
  • HCA Healthcare led the count of A hospitals with 51, followed by CommonSpirit Health with 49, AdventHealth with 29 and Kaiser Permanente with 27.
  • Utah had the highest share of A hospitals, with Virginia, New Jersey, Connecticut and North Carolina following, while Iowa, North Dakota, Vermont and Wyoming had none.
  • Disputes continued over methodology and impact, including a lawsuit from Tenet-owned Florida hospitals and questions about how the voluntary Leapfrog survey affects grades.
