Inquest Probes Nurse’s Death After Tameside A&E Sent Her Home With ‘Indigestion’

The coroner heard evidence of missed checks with absent risk documentation after she was routed to a same day unit.

Overview

  • Paula Ivers, 47, died on March 8, 2024 from a thoracic aortic dissection three days after being discharged from Tameside Hospital with a diagnosis of indigestion.
  • At the inquest, clinicians said her ECG was reviewed by consultant Dr Osama Ahmed, who did not examine her in person, with observations recorded as normal and chest X‑ray showing no acute findings.
  • The court heard her blood pressure was not measured in both arms and that there is no documented rationale for classifying her chest pain as low risk.
  • Coroner Christopher Morris questioned her placement in the Same Day Emergency Care unit, and Dr Ahmed agreed she was not low risk or suitable for that pathway.
  • Relatives alleged she was let down by the NHS, and the hearing was adjourned for further evidence at a date to be fixed.
