Inquest Probes Nurse’s Death After A&E Sent Her Home With Indigestion Diagnosis

The coroner heard the 'low risk' classification lacked supporting notes.

Overview

  • Paula Ivers, 47, died on March 8, 2024, from a thoracic aortic dissection that caused internal bleeding and cardiac arrest, a post-mortem found.
  • Three days earlier she went to Tameside Hospital with severe chest pain described as worse than childbirth, underwent blood tests, an ECG and a chest X-ray, and was discharged with a gastroesophageal reflux diagnosis.
  • A consultant, Dr Osama Ahmed, told the inquest he reviewed her ECG but did not examine her, was unaware of her family cardiac history, and accepted there was no documentation for deeming the case 'low risk'.
  • The coroner indicated she was not low risk or suitable for the Same Day Emergency Care pathway, while the family alleges failings in her assessment and diagnosis.
  • The inquest at Stockport Coroner's Court has been adjourned for further evidence to be heard at a later date.
