Overview
- Paula Ivers, 47, died on March 8, 2024, from a thoracic aortic dissection that led to internal bleeding and cardiac arrest.
- Three days earlier she attended Tameside A&E with severe chest pain, underwent blood tests, an ECG and a chest X-ray, and was discharged from the Same Day Emergency Care unit with a reflux diagnosis.
- Consultant Dr Osama Ahmed told the inquest he reviewed the ECG but did not examine her, was unaware of her family cardiac history, and recorded no justification for classifying her chest pain as low risk.
- Evidence heard said her observations, ECG and chest X-ray appeared normal, her blood pressure was not measured in both arms, and Dr Ahmed agreed she was not suitable for SDEC.
- Her family alleges failings in her care and Stockport Coroner’s Court will hear further evidence before deciding on any conclusions or recommendations.
