Incucai Updates Kidney-Transplant Rules to Speed Access

Aims to shorten waiting times by simplifying testing and paperwork to expand pre-dialysis transplants for patients who lost vascular access or are highly sensitized.

Overview

  • The new Resolución 375/2026, approved by Incucai's board on October 22 and published in the Boletín Oficial in late October, replaces rules from 2017 and 2021 to create a single, updated regulatory framework.
  • The rule standardizes criteria for preemptive transplants so patients can qualify for a kidney before starting dialysis based on common lab, biopsy, or imaging evidence of chronic kidney disease.
  • A new 'Último Acceso Vascular' priority recognizes people who have lost their final vein or catheter for hemodialysis and gives them defined standing on the national waiting list.
  • The Donación Renal Cruzada program will require the same immunological tests as conventional living donation to remove extra exams that had blocked exchanges and carried extra costs.
  • Incucai says the measures aim to cut administrative barriers but do not guarantee an organ for any patient; they act within a system of 7,352 people on the national list and 1,832 transplants recorded so far in 2026, and build on an August 2023 program that nearly doubled access for highly sensitized patients.
