Overview

  • Routine FSIS testing detected E. coli O26 in 90/10 ground beef produced on December 16 by Mountain West Food Group in Heyburn, Idaho (Est. 2083).
  • The recall covers 16-ounce vacuum-sealed packages labeled "Forward Farms Grass-Fed Ground Beef" with a use-or-freeze-by date of 01/13/26.
  • Shipments went to distributors serving California, Colorado, Idaho, Montana, Pennsylvania and Washington for retail sale.
  • FSIS is conducting recall-effectiveness checks and will post distribution details to verify removal from stores and consumer kitchens.
