Frazar made an 8-foot birdie on the first extra hole, marking his victory 12 years after his lone win on the PGA Tour at the St. Jude Classic.
Steve Stricker clinched his first Charles Schwab Cup title, despite not attending the tournament, securing his victory with two postseason events still to play.
Richard Green, who also closed with a 69, failed to birdie the 516-yard closing hole, which set up the opportunity for Frazar's victory.
