Kwan's placement on the IL is retroactive to September 14, following his absence from the lineup for the past five games.
To fill Kwan's roster spot, the Guardians have recalled outfielder Myles Straw from Triple-A Columbus.
Veteran pitcher Carlos Carrasco has been designated for assignment to make room for Straw on the 40-man roster.
Kwan, who has struggled in the second half of the season, is expected to return by September 24, with the Guardians leading the AL Central.
