• Lake Havasu Fire Department crews responded shortly before 11 p.m. July 9 to the 1800 block of Mesquite Avenue after a 911 call reported a burn patient inside a hyperbaric oxygen chamber.
• Firefighters encountered smoke throughout the building and discovered a flash fire had erupted inside the sealed chamber.
• Practitioner Walter Foxcroft, 43, was pronounced dead at the scene; no additional injuries were reported.
