• The Football Association (FA) has launched a formal investigation into the death of Sheffield United Women’s player Maddy Cusack, after receiving evidence from her family.
• Cusack's family alleges that her working relationship with Sheffield United’s women’s team manager, Jonathan Morgan, contributed to her mental health issues and subsequent death.
• An external investigation commissioned by Sheffield United found no evidence of wrongdoing by anybody connected to the club, including Morgan.
• Morgan, who temporarily stepped away from his post during the investigation, has returned to work at United.
• A coroner’s inquest into Cusack’s death has yet to formally conclude, and the police did not treat the circumstances as suspicious.
