FA Inquiry on Maddy Cusack’s Death to Be Shared with Coroner

The Football Association has completed its investigation into the 2023 death of Sheffield United player Maddy Cusack, with an inquest scheduled for April 2025.

Maddy Cusack, a Sheffield United midfielder, died in September 2023 at the age of 27, with her death not treated as suspicious by police.
The FA’s investigation into her death, launched in January 2024, has concluded and will be reviewed by the coroner this week.
The inquest will examine factors contributing to Cusack’s mental health deterioration, starting from February 2023, when Jonathan Morgan became Sheffield United’s head coach.
Cusack’s family had previously filed complaints against Morgan, citing a strained relationship that they believe impacted her mental health.
The inquest is set for April 16-17, 2025, with key witnesses including Morgan, Cusack’s family, and representatives from Sheffield United and the FA.
