• Police say Mario Green confronted his ex-wife, hospital employee Latricia Green, in a basement area on Aug. 22 and shot her multiple times before fleeing.
• He was taken into custody early Aug. 23 after security video showed him leaving and officers later found his 2011 white Dodge Charger abandoned.
• Court filings show she first sought a personal protection order on June 13, which was denied, and a second request granted on July 21 was not served to him.
• Wayne County permits service of protective orders by process server, court officer, third party or certified mail, but police said the July order was not delivered.
• Henry Ford Health said it is cooperating with Detroit police and conducting its own internal review, and the hospital has reopened as investigators examine how the shooter accessed the basement.
