Overview
- Martina Karos, 40, and her eight-year-old daughter, Eleni Edwards, were discovered at their Kersal home on 23 September 2024 after emergency services forced entry when Eleni missed school.

- Area coroner Peter Sigee said Karos deliberately created the fatal environment, citing overwhelming loneliness despite support, and stated the incident could not reasonably have been foreseen.
- Agencies described a comprehensive package of help, including 247 respite visits over 14 months and an active Child Protection Plan with a review set for the day they were found, with Salford council saying local resources were exhausted.
- Following rapid and full safeguarding reviews, six recommendations were made, and agencies reported changes including a new referral and assessment hub plus strengthened multi-agency recording and safeguarding training.
