  • Rhianan Rudd was groomed from age 14 by American neo-Nazis Dax Mallaburn and Chris Cook, who introduced her to extremist ideology and bomb-making instructions.
  • At 14 she became the youngest person in the UK charged with terrorism offences and was investigated by MI5 and counter-terror police before charges were dropped in December 2022.
  • The inquest found that counter-terrorism police and Derbyshire County Council failed to refer her early to the National Referral Mechanism and to Nottinghamshire CAMHS.
  • Judge Durran ruled that these missed interventions were individual errors in an otherwise functioning system and declined to issue a prevention of future deaths report.
