CDC Adds Detransition and Transition-History Codes to U.S. ICD-10-CM

Effective Oct. 1, 2026, the change will put detransition into medical records, enable billing, allow data collection, prompt new research.

Overview

  • The Centers for Disease Control and Prevention announced on Aug. 18 that new ICD-10-CM codes will formally recognize detransition and personal histories of social, medical, and surgical gender transition.
  • The updates include a remission code listed as F64.A and new Z87.* codes for personal history of social, medical, surgical transition and for gender detransition to support clinical recordkeeping and billing.
  • Do No Harm says it proposed the specific codes and called the rollout a win for detransitioners, but its prevalence claims about thousands of children who received gender‑affirming treatments have not been independently verified.
  • The code additions do not change clinical guidelines or guarantee insurer coverage by themselves, though clinicians, payers and researchers can use the codes to track outcomes and claims data.
  • Conservative outlets and the advocacy group framed the decision as evidence of harms from pediatric gender care, while other reporting notes the CDC had not answered all media inquiries and that the data’s policy effects remain to be seen.
