- Aichi Prefecture announced on Feb. 10 that a review found 1,034 instances of inappropriate processing at its Mental Health and Welfare Center during fiscal 2023–24.
- A 27-year-old male employee assigned from the Toyokawa Health Center failed to obtain required approvals, falsified examination results, and altered disability grades to heavier levels.
- The tally includes 253 mental disability handbooks and 781 self-support medical recipient certificates used for outpatient cost reductions.
- In interviews with the prefecture, the employee said he changed grades because work had fallen behind and he aimed for outcomes applicants would accept.
- The prefecture says most cases were later regularized through proper approvals or renewals and it is considering disciplinary action for the employee.
