AAIB Blames Unstabilised Approach for 2023 Mumbai Learjet Crash

The final report calls for a one-time DGCA audit of fixed-wing training records and for stall-recovery in landing configuration to be added to recurrent simulator checks.

Overview

  • The Learjet 45XR operated by VSR Ventures crashed while landing at Mumbai on September 14, 2023, leaving the aircraft destroyed and all eight people on board seriously injured.
  • AAIB says the probable cause was the crew’s decision to continue an unstabilised manual approach in marginal weather and inappropriate handling that led to an aerodynamic stall at very low altitude.
  • Cockpit voice recorder transcripts show the captain disconnected autopilot, visual contact was lost, a late go-around call came at about 120 feet, stall warnings activated and the jet stalled before striking grass near taxiways.
  • Investigators found systemic operator failures at VSR: recurrent simulator checks omitted mandatory emergency items, technical and airframe logbooks were incomplete and internal audits were not carried out diligently.
  • The bureau published its final report on October 16–17, 2026 and recommended a one-time DGCA review of training records, mandatory stall-recovery training in landing configuration, stricter go-around enforcement and improved ATC separation procedures, placing VSR and regulators under intensified scrutiny.
